Please enter your last name, first name, and section below, then , for each performance date, select "Yes", "No", or "Not Sure Yet". Please provide a comment if you are responding "Not Sure Yet" to a performance that is less than 3 WEEKS away. When you are finished, click the "Submit" button. Update the form as often as needed.
